COPD is primarily caused by smoking (90% of cases) and air pollution. Existing damage cannot be fully reversed, but stopping smoking immediately slows progression and improves outcomes. Medications, pulmonary rehabilitation, and sometimes oxygen therapy maintain quality of life.
